Center for Hospice Care believes every person, every story deserves dignity, compassion and support. Our continuum of care supports patients and families through every stage of life.

From the very beginning, we provide specialized prenatal and perinatal palliative care for families facing life-limiting diagnoses. Our team offers
emotional support and compassionate care for both parents and infants.

For children and adolescents, our pediatric palliative care program focuses on improving the quality of living for young patients, like Adler, with serious or chronic conditions. We help families manage symptoms, provide emotional support and create moments of joy.

For adults living with dementia, Milton Village offer a safe, engaging environment that promotes social connection and cognitive stimulation, while providing caregivers the respite they need.

For people with serious advanced illnesses, our home-based Kaleidoscope palliative care team delivers expert symptom management, caregiver support and education, helping patients live fully and comfortably wherever they call home.

Our hospice program surrounds patients and families with 24/7 support, symptom relief, and emotional and spiritual care. We continue to walk with families through grief and healing with bereavement services offered by our Life Transition Center.
