There are many ways to support the Hospice Foundation, from giving a memorial gift in memory of a loved one to various naming opportunities. Choose from the options below to ensure that your donation is properly allocated.

Donations to the Area of Greatest Need give us the greatest flexibility to use the funds on our most critical projects or initiatives.

Hospice Foundation’s 2023 Annual Appeal – “Care Throughout the Circle of Life” will make sure that anybody eligible for hospice care will never be turned away, regardless of their ability to pay.

To meet the hospice and palliative care needs of our community now and in the future, we constructed our Mishawaka campus – including the Ernestine M. Raclin House (a 12-bed inpatient unit on the banks of the scenic St. Joseph River), a centralized facility for doctors, nurses and other clinical staff supporting CHC patients and our administrative building that houses CHC’s admissions, IT, accounting, and administrative teams as well as the Hospice Foundation and the Life Transition Center.

Since 1980, Center for Hospice Care has kept its promise that no one eligible for hospice care will be turned away due to an inability to pay. Each year, CHC spends millions of dollars on charity care. To support this mission and to honor a long-serving member of our community, the Hospice Foundation has created the Sister Carmel Helping Hands Fund.

Those who would like to donate an item honor or memory of a loved one, have many opportunities to do so.

In addition to providing compassionate end-of-life care to patients, Center for Hospice Care provides programs to grieving family members – at no cost. The Hospice Foundation is proud to help raise support to ensure these programs are available to all in our community in perpetuity.

  • Donate to Camp Evergreen – A camp for grieving children that gives them the opportunity to share their grief with other kids, and to have fun participating in camp activities.
  • Donate to After Images – A bereavement program that uses a combination of art and counseling to help participants process their grief.

Center for Hospice Care is proud to be part of the “We Honor Veterans” campaign, a collaboration between the Department of Veterans Affairs and the National Hospice and Palliative Care Organization. Its purpose is to raise awareness among veterans of the end-of-life services and benefits available to them.

Center for Hospice Care (CHC) is very proud to announce we have attained Level 5 of the We Honor Veterans program. Not only does it reinforce our commitment to veterans and the unique end-of-life issues they may have, but it also establishes us as a mentor for other hospice organizations. CHC is one of 170 hospices (out of +3,300 partners) to have attained this level since it was announced in February 2019.

By endowing this important program, CHC can continue to provide what may be the last opportunity for veterans to feel that their service was not in vain, and that they are appreciated.

Thanks to the Vera Z. Dwyer Charitable Trust and generous donors, CHC has been able to fund an additional seat in the IU School of Medicine’s one-year Fellowship in Hospice and Palliative Medicine program. This program is dedicated to training individuals to develop expertise in the care of patients with serious illness and to become leaders in the field of palliative medicine.

DAF Direct allows donors to recommend grants from their donor-advised fund directly from our website without incurring any download or transaction fees.

For several years, Center for Hospice Care and the Hospice Foundation have partnered with the Palliative Care Association of Uganda to help improve the availability and quality of hospice care across Uganda. Road to Hope was created to provide critical support to children who are orphaned after serving as caregivers for the terminally ill parent.

A bequest or other planned gift to the Hospice Foundation leaves a legacy of caring that extends beyond your lifetime. The opportunities available may allow you to make significant gifts with favorable tax consequences. Investing money in a way that allows you to receive benefits during your lifetime while the remaining funds go to the Hospice Foundation after your death is another possibility.

The Hospice Foundation has partnered with FreeWill to give you a free tool to create your will and legacy at no cost. You can use this resource on its own or use it in conjunction with an attorney or financial planner to determine your needs and how best to make a planned gift. Giving to Hospice Foundation can include donations other than cash. Giving property or other material goods can benefit both the giver and Hospice Foundation as it supports the work of Center for Hospice Care. The IRS has specific regulations dealing with gifts in kind that are more than $5,000. We suggest companies work closely with a tax advisor in planning in-kind gifts of this size or larger.

Workplace giving programs benefit not only Center for Hospice Care, but also your organization and its employees. Organizations build employee engagement and morale, boost the community in which they operate and are viewed as good corporate citizens.
